29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made, aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard, and permanent aisles and passageways were not appropriately marked: a) Jobsite, 516 Delwar Rod, Pittsburgh, PA: On or about May 20, 2013, employees operated heavy mechanical handling equipment on the "shredder road" to and from the upper mill to the lower mill, and were exposed to potential hazards due to the unstable, rough road conditions, the sometimes overloaded fluff pile that spilled over onto the road creating obstructions and the lack of markings to designate the edges of the road and the fluff pile.
