FIVE STAR QUALITY CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION# 699658, CITATION# 1, ITEM# 1, AND WAS AFFIRMED AS A FINAL ORDER ON 4/22/13, WITH RESPECT TO A WORKPLACE LOCATED AT 1815 LOMBARD STREET, PHILADELPHIA, PA.    FIVE STAR QUALITY CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION# 312494545, CITATION# 1, ITEM# 5, AND WAS AFFIRMED AS A FINAL ORDER ON 5/23/13, WITH RESPECT TO A WORKPLACE LOCATED AT 125 EAST MAIN STREET, MARLTON, NJ.
