29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501. a) Jobsite: 1600 South St., Philadelphia, PA - On or about 5/20/13, no conventional or alternative means of fall protection was used to protect employees from falling off the unprotected open sides and edges of the residential construction building. The employees were performing framing operations approximately 38 feet above ground level. FIVE STAR QUALITY CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION# 699658, CITATION# 1, ITEM# 1, AND WAS AFFIRMED AS A FINAL ORDER ON 4/22/13, WITH RESPECT TO A WORKPLACE LOCATED AT 1815 LOMBARD STREET, PHILADELPHIA, PA. FIVE STAR QUALITY CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION# 312494545, CITATION# 1, ITEM# 5, AND WAS AFFIRMED AS A FINAL ORDER ON 5/23/13, WITH RESPECT TO A WORKPLACE LOCATED AT 125 EAST MAIN STREET, MARLTON, NJ.
