29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) Job site, 254 & 256 Fifth Avenue, Pittsburgh, PA: On or about May 20, 2013, during building restoration activities, the employer did not ensure that scafolding was properly inspected. Deficiencies found during the inspection, included, but were not limited to, a missing midrail, where an approximate 25 foot fall hazard existed.
