29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): Location: 1808 Schulte Hill Drive, Maryland Heights, MO 63043 Free Flo Gutters Inc. allows workers to be exposed to hazardous conditions created by not conducting frequent or regular inspections of the worksite to ensure all workers were in compliance with OSHA standards. The workers were exposed to a fall hazard that measured 11 feet, 11 inches at the northeast corner and 7 feet, 8 inches at the southwest corner as employees worked from the roof to install aluminum gutters without the use of an adequate fall protection system and/or fall arrest system. In addition, workers were exposed to an electrical hazard as they removed and installed aluminum gutters within approximately 2 feet of the energized electrical service drop and weather head at the southeast corner of the residential structure.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: Two employees gained access and egress by utilizing a portable ladder which did not extend at least 36" above the upper landing (roof). The side rails measured approximately 24" above the landing. This improperly positioned ladder was located at the roof rake along the south side of the residential structure. The fall hazard at the southwest eave measured approximately 7 feet, 8 inches.
