29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Trench, new residential construction located at 634 Concord Lake Circle, Smyrna, GA 30082: Employees installing a 6-inch sewer line in a 6-feet wide by 25-feet long by 10-feet deep trench were not provided head protection. A cave-in protective system was not provided by the employer as employees were exposed to possible soil collapse with no hard hats inside an excavation.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Trench, new residential construction located at 634 Concord Lake Circle, Smyrna, GA 30082: Employees worked in an unprotected trench which measured 6-feet wide by 25-feet long by 10-feet deep without a safe means of egress, i.e. ladder and or ramp. Employees exposed to possible soil cave-in or collapse of the trench walls were not provides a safe means of egress.
