A) 774 Route 1, Iselin, NJ 08830: Employees were exposed to fall hazards of approximately 16' from the tube and coupler scaffold platform in which a fall protection system was not erected or used by employees engaged in demolition prior to exterior stucco work. The tube and coupler scaffold work platform was missing mid and end rails. Condition occurred on or about May 21, 2013.
