29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: Employees sandblasting and spray painting are required to wear a sandblasting hood and a 3M 3500 tight-fitting, cartridge type, half mask respirator. A written respiratory protection program had not been established by the employer that contained: (i) Procedures for selecting respirators for use in the workplace; (ii) Medical evaluations of employees required to use the respirators; (iii) Fit testing procedures for tight-fitting respirators; (iv) Procedures for proper use of respirators in routine and reasonably foreseeable emergency situations; (v) Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators; (vi) Procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators; (vii) Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations; (viii) Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance; and (ix) Procedures for regularly evaluating the effectiveness of the program.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200 will be met: Employees were exposed to chemical hazards when using propylene, oxygen, industrial paint, commercial grade sand and other chemicals. The written hazard communication program did not address all of the requirements of the standard. The written hazard communication program did not include: a. The methods the employer will use to inform employees of the hazards of non-routine tasks the employer assigns employees. b. The methods the employer will use to provide the other employers on-site access to material safety data sheets for each hazardous chemical the employees may be exposed to while working. c. The methods the employer will use to inform the other employers of any precautionary measures that need to be taken to protect employees during the workplace's normal operating conditions and in foreseeable emergencies. d. The methods the employer will use to inform the other employers of the labeling system used in the workplace. e. The methods the employer use to provide employee information on hazardous chemicals
