29 CFR 1910.147(c)(4)(ii): Energy Control Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance, including but not limited to (A) through (D) of this section: On or about May 14, 2013, and times prior thereto, on SACMI PH690 Press #1, the employer did not develop energy control procedures that contained specific procedural steps to shut down, isolate, block or secure all sources of energy, and did not develop adequate means to verify the effectiveness of the energy control measure. This condition exposed employees to struck-by or caught-in hazards due to unexpected machine movement and the release of stored energy.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, were not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about May 31, 2013, and times prior thereto, employees are exposed to caught-by, struck-by hazards in the point of operation of presser No.1 located by the face of the presser where safety bar (left and right side of the presser) connects to the presser. This condition exposed employees to caught-by, struck-by safety hazards.
