29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: On or about May 22, 2013, at the above address jobsite, employees using a fiberglass extension ladder to gain access to and from a low slope roof approximately 18 feet above a lower level set up in an active parking lot did not have that ladder secured or barricaded to prevent accidental displacement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: On or about May 22, 2013, at the above address jobsite, employees performing roof work installing insulation on a low slope roof with unprotected edges approximately 17 ft. above a lower level were not protected from falling to that lower level by the use of guardrail systems, safety net systems and or personal fall arrest systems. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVILANT STANDARD 29 CFR 1926.501(B)(11), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313106627, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON 03/01/2010, WITH RESPECT TO A WORKPLACE LOCATED AT 9050 AMERICAN WAY, VERO BEACH, FLORIDA.
