29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  The employer does not ensure that the scaffold was fully planked during construction work.  This violation was observed on or about May 21, 2013, on the south side of a commercial building where employees were exposed to a fall hazard when installing metal flashing material without a fully planked scaffold.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each platform on all working levels of scaffolds are fully planked or decked between the front uprights and the guardrail supports as specidied in paragraphs 1926.451(b)(1)(i)-(ii).

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  The employer does not provide employees with access to the scaffold to the upper level of the scaffold.  This violation was observed on or about May 21, 2013, on the east side of a newly built commercial building where employees were exposed to a fall hazard when accessing the scaffold platform at a height of 15 feet without a ladder.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that when scaffold platforms are 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders or similar surface is used.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  The employer does not install guard rails on the scaffold during construction work. This violation was observed on or about May 21, 2013, on a newly built commercial building where employees were exposed to a fall hazard when required to do masonry work from a platform of 15 feet without a guard rail system . Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guardrail systems are install along open sides and ends of platforms.
