Harold O'Shea Builders, Inc., Springfield, Illinois: On or about May 22, 2013 the employer did not provide the required injury or illness descriptions on the OSHA 300 Log for the calendar year 2013. a) On or about, May 14, 2013, a recordable injury occured when an employee was adjusting ther rigging on a precast floor beam. the floor beam fell with the employee on it, resulting in a broken toe, contusion and swelling of the left knee. The description in column F did not describe the injury or the object that caused the injuries. b) On or about, May 14, 2013, a recordable injury occured when an employee was adjusting the rigging on a precast floor beam. The floor beam fell with the employee on it, resulting in 2 days of on the job transfer or restriction. The description in column F did not describe the injury or the object that caused the injuries.
