29 CFR 1926.600(a)(6)(i): For lines rated 50kv or below, a minimum of 10 feet was not maintained between the energized line and the equipment.  On or about May 20, 2013, and at times prior thereto, the employer did not ensure that its equipment operators and sub-contractors maintained all parts of hauling/dumping equipment at least ten feet away from overhead power lines, exposing the workers to electrical hazards.
