The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  WORKSITE: The employees exposed to fall hazards while performing roofing activities were not provided with training in the recognition of hazards associated with falls or the procedures necessary to eliminate of minimize those hazards.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  WORKSITE: The Werner 20 foot aluminum extension ladder, used by employees to access the roof was setup at an angle where the horizontal distance from the top support to the foot of the ladder exceeded one-quarter (1/4) the of the working length of the ladder.

Construction LLC was previously cited for a violation of this occupational safety and health standard (29 CFR 1926.501 (b)(13)), which was contained in OSHA inspection number 315720243, citation number 2, item number 1, affirmed as a final order on 1/20/2012, with respect to a workplace located at 27 Glen Road, Newtown, CT.     L.L.E. Construction LLC was previously cited for a violation of this occupational safety and health standard (29 CFR 1926.501 (b)(13)), which was contained in OSHA inspection number 109178012, citation number 1, item number 1, affirmed as a final order on 11/29/2010, with respect to a workplace located at 460 East Main Street, Bridgeport, CT.    L.L.E. Construction LLC was previously cited for a violation of this occupational safety and health standard (29 CFR 1926.501 (b)(13)), which was contained in OSHA inspection number 311319487, citation number 1, item number 2, affirmed as a final order on 10/30/2009, with respect to a workplace located at 780 Seaview Avenue, Bridgeport, CT.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  WORKSITE: The side rails of the Werner 20 foot, type III aluminum extension ladder, used by employees during the roofing activities did not extend at least 3 feet above the eave of the roof.     L.L.E. Construction LLC was previously cited for a violation of this occupational safety and health standard (29 CFR 1926.10531 (b)(1)), which was contained in OSHA inspection number 109178012, citation number 1, item number 2a, affirmed as a final order on 11/29/2010, with respect to a workplace located at 460 East Main Street, Bridgeport, CT.
