29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about May 20, 2013, and in times prior thereto, the employer did not train workers to recognize the hazards of permitting equipment with raised dump bodies to be operated within 10 feet of overhead power lines, exposing employees to the hazards of electrical shock.

29 CFR 1926.600(a)(6)(i): For lines rated 50kv or below, a minimum of 10 feet was not maintained between the energized line and the equipment. On or about May 20, 2013, and at times prior thereto, the employer did not ensure that equipment operators maintained all parts of hauling/dumping equipment at least ten feet away from overhead power lines, exposing operators and nearby pedestrians to electrical hazards.
