Capacity, operation, and maintenance plates, tags or decals of the powered industrial truck were not changed accordingly:   Maintenance Shop Yard:  On, or about, May 22, 2013, the CAT forktruck, serial number AT87A01366, was provided with eight foot long forks.  The identification plate mounted on the vehicle showed a load capacity based on four foot long forks.

Maintenance Shop, Lower East Container Storage:  A Siemens circuit breaker box located at the back of the container was blocked by buckets, wood, a fan and pane of vehicle glass.    B.     Processing Facility, Off-Loading Bay:  A concrete barrier had been placed in front of the Murray circuit breaker box and the GE circuit breaker box to protect them from vehicular damage.  C.      Processing Facility, Plasma Cutting Work Area:  Shelving had been installed under and around the GE circuit breaker box.  D.     Processing Facility, Boiler Room:  A barrel of soap, pressure washing unit and shelving was located in front of and on the side of the GE circuit breaker box.
