29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) Colorado Roofing & Waterproof, LLC @ 530 Grand Ave., Grand Junction, CO. On or before April, 19, 2013, the exposing employer did not ensure that each employee was tied off to the boom or basket when working from an extensible articulating elevated aerial lift. This condition exposed employees to a fall hazard of approximately 15 feet to ground. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part 1926.502(d) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a)Colorado Roofing & Waterproof, LLC @ 530 Grand Ave., Grand Junction, CO. On or before April, 19, 2013, the exposing employer did not ensure that each employee performing activities on the roof(s) of the building under construction utilized fall protection systems to prevent fall from elevation. This condition exposed employees to the hazard of falling 6 feet or more to lower levels. (b)Colorado Roofing & Waterproof, LLC @ 530 Grand Ave., Grand Junction, CO. On or before April, 19, 2013, the exposing employer did not to ensure a fall protection system were in place at each unprotected side(s) and edge(s) used by employees as point of access to the first level walking/ working roof(s) surface where no guardrail and/or other systems were erected to protect employees from a fall hazard of approximately 11 feet to ground.
