29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. (a) Greeley Washout, Inc, 1140 E. 8th Street, Greeley, CO 80631: On and before May 23, 2013, the employer did not provide a guardrail to protect employees from falling into the open washout ditch. The work area contains a 2-3 foot drop into a 7 foot deep washout ditch. This condition exposed employees to fall hazards.

29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentially walk was not guarded by a floor hole cover of standard strength and construction: (a) Greeley Washout, Inc, 1140 E. 8th Street, Greeley, CO 80631: On and before May 23, 2013, the employer did not guard a floor hole that was 10 inches wide in diameter. An employee fell in this hole located next to ramp 1. This condition exposed employees to trip and fall hazards.
