29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  Location:  918 Townhouse Lane, Hazelwood, MO  63042  D and K Roofing allows workers to be exposed to hazardous conditions created by not conducting frequent or regular inspections.  The most recent occurrence was at 918 Townhouse Lane, Hazelwood, MO  63042 where the employer failed to make frequent or regular inspections of the work site to ensure all workers were in compliance with company safety and health policies and OSHA standards.  The workers were exposed to an approximate 9 foot fall hazard as they worked on the roof without the use of an adequate fall protection system and/or fall arrest system.  In addition, workers were exposed to struck-by hazard as they used nail guns without donning the necessary eye protection.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  Location:  918 Townhouse Lane, Hazelwood, MO  63042  Workers were using nail guns to install shingles on the roof of the residential structure without donning the necessary eye protection.  D and K Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 451994, citation number 1, item number 3 and was affirmed as a final order on 11/27/2012, with respect to a workplace located at 11814 Old St. Charles Rock Road, Bridgeton, MO  63044.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  Location:  918 Townhouse Lane, Hazelwood, MO  63042  Workers were actively shingling a low slope roof without the use of any fall protection system.  The workers were exposed to a 9 foot fall to the ground below.  D and K Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 451994, citation number 1, item number 5a and was affirmed as a final order on 11/27/20012, with respect to a workplace located at 11814 Old St. Charles Road, Bridgeton, MO  63044.
