29 CFR 1910.255(b)(4): All press welding machine operations, where there was a possibility of the operator's fingers being under the point of operation, were not effectively guarded by use of a device such as an electronic eye safety circuit, two hand controls:    a) On or about April 1, 2013, located in the wire welding department the spot/projection welder serial number #50984 was not effectively guarded by distance, in that the operator utilized a foot control switch in part of the wire welding process that was set up to close to the machine allowing the operator fingers and hand extremities to reach and to be caught between the part and the closing welding clamps.
