29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      a)  Worksite, 2 E. Main St., Kings Park NY - An Employee was not wearing a protective helmet and was exposed to being struck by overhead materials and tools while working below overhead work; on or about 05/24/13. The employee was standing on a single 2x8in plank that was set up on the ladder; on or about; 5/24/13.
