29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph: a) On or about May 23, 2013, two (2) employees working on a pump jack scaffold did not have adequate fall protection in that there was no midrail and the ends of the scaffold did not have a guardrails installed. Employees were exposed to fall hazards in excess of 15 feet. Abatement Note: Abatement certification and documentation are required for this item.
