29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:     a) On or about May 23, 2013, the employer did not ensure that employees were using proper personal protective equipment.  Employees were installing wood planks with pneumatic nails without using adequate eye protection.                   Abatement Note:   Abatement certification is required for this item. 29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph:      a) On or about May 23, 2013, two (2) employees working on a pump jack scaffold did not have adequate fall protection in that there was no midrail and the ends of the scaffold did not have a guardrails installed.  Employees were exposed to fall hazards in excess of 15 feet.       Abatement Note:   Abatement certification and documentation are required for this item.
