29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a) Throughout the Building:  Employees pouring and/or spraying various hazardous chemicals including but not limited to bleach (sodium hypochlorite), Sheila Shine (perchlorethylene), Spray Nine (2-butoxyethanol), Non-Ammoniated Stripper (potassium hydroxide) etc. were not required to wear appropriate eye protection (e.g., chemical splash goggles); on or about 5/21/13.
