29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    On or about May 21, 2013 at the above addressed jobsite, an employee was exposed to a fall hazard while performing roofing operations approximately 21.7 feet above the ground. The employee was not utilizing fall protection.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  On or about May 21, 2013 at the above addressed jobsite, an employee was using a ladder to access the roof. The ladder did not extend three feet above the landing surface and the employee was exposed to fall hazards associated with ladders.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
