29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.      A.      On or about May 23, 2013, employees who did not tie-off to a personal fall arrest system were exposed to a fall hazard where open panels in the roof were located.  The fall distance to the concrete surface below was approximately 22 feet.     B.      On or about May 23, 2012, employees were exposed to a fall hazard.  Warning lines, or other suitable methods, were not installed to establish boundaries for areas such as perimeter roof panels that were considered unsafe for employees to stand on.  The fall distance to the concrete surface below was approximately 22 feet.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge.      On or about May 23, 2013, employees were exposed to a fall hazard where open panels in the roof were located.  A warning line system made from plastic barrier tape was not maintained at all times to provide at least 6 feet (1.8m) of surface space between the line and open roof holes.  The fall distance to the concrete surface below was approximately 22 feet.

29 CFR 1926.502(f)(2)(i): Warning lines did not consist of ropes, wires, or chains and supporting stanchions that were flagged at not more than 6-foot (1.8m) intervals with high visibility material.    On or about May 23, 2013, employees were exposed to a fall hazard where open holes in the roof were located.  A warning line system, that demarcated the open hole fall hazards, consisted of red plastic barrier tape that was tied to the roof channel structures.  The fall distance to the concrete surface below was approximately 22 feet.

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface.      On or about May 23, 2013, employees were exposed to a fall hazard where open holes in the roof were located.  A warning line system was made by tying plastic barrier tape to the roof channel structures.  The placement of the barrier tape resulted in a warning line that was approximately 13 inches above the walking/working surface.  The fall distance to the concrete surface below was approximately 22 feet.

29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard.      On or about May 23, 2013, employees were exposed to a fall hazard where open holes in the roof were located.  Plywood sheets were used to cover holes where glass panels were absent.  Weather-proof sheets that covered the installed plywood lacked labels identifying the underlying holes.  The fall distance to the concrete surface below was approximately 22 feet.

On or about May 23, 2013, employees were exposed to a fall hazard where open panels in the roof were located.  Supervisors were assigned a dual function to monitor production activities of roof work and to monitor the controlled access zone to prevent unauthorized workers from entering the leading edge work area.  The fall distance to the concrete surface below was approximately 22 feet.
