(a) Pagoda Village Apartments Jackson, MS - On or about May 21, 2013, employees engaged in re-roofing two apartment buildings with a roof slopes of 5:12, were exposed to falling 17.6 to 27 feet due to the employer's failure to provide personal fall-arrest equipment.
