On or about 5/22/13, at the Oakwood facility, employees engaged in service/maintenance to the "Site Pipe" machine on cell #6 in the production area were not utiliziing locks on the machine as a part of the LOTO procedure established by the employer.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  On or about 5/22/2013, employees performing service to the "Site Pipe" machine on Cell #6 in the Oakwood production area had not received training on the specific written lockout procedures for that machine. Authorized employees had training on procedures at the Santa Rosa, Mexico facility; however, procedures were different than those established at the Oakwood facility.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
