29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: Plant , Panel Area - On or about 05/23/13 the emergency exit door adjacent the saw station was obstructed by 3.5 in. x 6 in. galvanized steel beams that were placed in storage. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Plant - On or about 05/23/13 employees performed lockout/tagout of equipment including (but not limited to) the Scotchman dual operator 120/200 Iron Worker, the Scotchman Model FI.85/140 Iron Worker, the Peddinghaus Corporation Angle Shear Machine, and the Ocean Machinery, Inc. Beam Driller. The employer did not develop, implement, and utilize individual written lockout/tagout procedures to protect employees from electrical energy, compressed air, hydraulic fluid systems, and the sudden activation of moving/rotating parts. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
