29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:(a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY) (a) North Side of Building - On or about May 24, 2013 employees were exposed to electric shock hazards while using a flexible cord during the installation of conveyors. Employees were not protected by a continuous ground through the flexible cord.
