29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper surface to which the ladder was used to gain access. A) 66 Hagaman Street, Woodbridge, NJ 07095: The ladder provided was not secured and did not extend 3 feet above the working surface of the roof to allow for safe access on and off the roof. Condition observed on or about May 14, 2013.
