29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:     On or about May 22, 2013, outside of the Top Doghouse, the 480V nominal electrical cord supplying power the Top Dog House had its outer insulation jacket sliced opened. One conductor inside was damaged exposing bare wire. Employees were exposed to electrical shock hazards.
