29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s). Monopod Platform, motor room; on or about 21 May 2013 and times prior thereto, the employer did not ensure that lock-out tag-out program provided the identity of the individual that applied the tag. The Notice tag located in the motor room for the air compressor # 3; was not marked identifying the individual that applied the tag. This condition exposed employees to inadvertent start-up of the equipment resulting in caught-in hazards.
