Anna Platform, Galley and Housekeeping: On or about May 20, 2013 and at times prior thereto, the employer did not provide a written personal protective equipment hazard assessment certification of the workplace evaluated and the person who conducted the evaluation. Certification ensures employees exposed to hazards are provided with the appropriate personal protective equipment.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met. Anna Platform, Galley and Housekeeping: On or about May 21, 2013 and at times prior thereto, the employer had not developed a written hazard communication program. This condition exposed employees to chemical hazards in the workplace including cleaning chemicals such as 409, Windex, and bleach.
