29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about May 21, 2013, two employees were using an aerial lift without wearing body belts and lanyards attached to the basket. They were plastering the outside of a residential building. The employees were exposed to a potential fall hazard of more than 10 feet to the ground below.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about May 21, 2013, two employees were working off an aerial lift. The employer did not ensure that all employees had been trained on how to work on an aerial lift.
