29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    On or about May 23, 2013, In front of the house, three instances where the employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees. Employees were exposed to electrical hazards.      a)    In front of the house, a 100-foot red/black 16 AWG extension cord plugged into the electrical post, used to power other extension cords had its strain relief busted at the female end and the ground plug removed at the male end.      b)    In front of the house, a 100-foot orange/black 16 AWG extension cord used to power the 7 1/4-inch DeWalt Circular Saw (model DWE 575, serial #142235) had strain relief busted at the female end.      c)     Inside the house, a 100-foot orange 16 AWG extension cord used to power the Homelite electrical blower had insulation damage.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:      On or about May 23, 2013, three employees were observed constructing a roof deck without utilizing any fall protection system. Employees were exposed to a fall hazard of greater than six feet.      a)    First employee observed working at the back of the house installing sheathing at the highest point of the pitched roof. The employer stated that the highest point of the pitched roof measured approximately 22-feet.      b)    Second employee observed working in front of the house at the leading edge and near the entrance.       c)    Third employee observed installing sheathing over the garage.
