29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: On May 28 2013, at this job site, employees were exposed to a fall from 10 feet when using a Werner extension ladder to access the top side of a residential roof in which the side rails did not extend at least 3 feet above the landing of the roof.
