29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On June 28, 2013, at this construction site, employees were exposed to a fall hazard from approximately 9 feet to the ground level below while conducting roofing work without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  On May 28 2013, at this job site, employees were exposed to a fall from 10 feet when using a Werner extension ladder to access the top side of a residential roof in which the side rails did not extend at least 3 feet above the landing of the roof.
