29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means. a) Delaware Avenue and St. Luke's Place, Worksite - on or about May 28, 2013, employees were conducting excavating work using a backhoe which struck and ruptured a previously marked underground gas line but its exact location was not determined by safe and acceptable means.
