29 CFR 1926.350(a)(6): Compressed gas cylinders were being moved without being secured on a special carrier intended for this purpose and did not have the regulators removed and valve protection caps put in place: On or about May 22, 2013, on the bed of the boom truck, oxygen and acetylene cylinders were stored horizonatally, with welding attachments left on and valve protection caps not utilized, exposing employees to fire and struck-by hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about May 22, 2013, two employees were observed working off a Boom Truck Aerial Lift approximately 7-feet above the next lower platform without any form of fall protection. The employees then moved the Aerial Lift away from the lower platform exposing themselves to a fall approximately 14-feet to the ground.
