29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:   Employees were exposed to fall hazards of approximately 17.9 feet and 14.3 feet above the ground below while installing flashing because of they were not utilizing fall protection.

29 CFR 1926.503(a)(1):   The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   Employees were exposed to fall hazards while performing roofing activities at heights of 17.9 feet and 14.3 feet above the ground below due to the lack of fall protection and the employer did not have a training program for providing fall protection for the employees.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface to which the ladder was used to gain access:   Employees were exposed to fall hazards because the aluminum extension ladder used to access the roof did not extend three feet above the roof.  The employee was working approximately 14.3 feet above the ground below.

29 CFR 1926.1060(a)(1)(iii):   The employer did not ensure that each employee using ladders and stairways had been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders:   Employees were exposed to fall hazards because the aluminum extension ladder did not extend three feet above the roof and the employer had not ensured that each employee had been trained by a competent person.
