29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about May 21, 2013, one employee observed working in front, along the edge and near the entrance of the house was constructing a roof deck without utilizing any fall protection system. The employee was exposed to a fall hazard of greater than six feet.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: On or about May 21, 2013, In front of the house, a 100-foot red/black 16 AWG extension cord plugged into a Honda Blackmax, Model GX 390 OHV, 8450 Maximum Watt, portable generator used to power the 7 1/4-inch Skil Circular Saw (Model/Serial No. un-identifiable) had outer insulation damage. Employees were exposed to electrical hazards.
