29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) 1031 Voorhees Drive, Voorhees, NJ; west side of building - Employer's representative did not recognized basic unsafe conditions in the setup of the fabricated frame scaffold used on site by the subcontractor (no fall protection or improper fall protection by employees working on the scaffold, unsafe means of access for upper levels of scaffold, inadequate overlap of platform planks - less than 6 inches, spacing between planks greater than 1 inch), on or about 05/10/13. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. Elite Stucco Design, Inc. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 29 CFR 1926.454(a) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312503915, CITATION NUMBER 01, ITEM NUMBER 005, ISSUED ON 06/29/2009.
