29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On May 28, 2013 an employee was not protected from a cave-in while working in a unprotected trench, exposing the employee to the hazards associated with a cave-in. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
