29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about May 21, 2013, two employees were observed working along the edge of the building on a flat roof installing roof sheathing without any form of fall protection, employees were exposed to a fall hazard exceeding 6 feet.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii): On or about May 21, 2013, two employees were observed working along the edge of the building installing roof sheathing without any form of fall protection. The employer did not assure that each employee exposed to fall hazards was trained in the recognition and avoidance of unsafe conditions. Employees were exposed to fall hazard exceeding 6 feet.
