OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that workers were exposed to fall hazards: (a) Clean side of Production: On or about May 6, 2013, and at times prior, workers were exposed to fall hazards when they leaned a 10 foot Werner step ladder against dryer number 5 to access an upper level more than 11 feet high which caused the workers to use an unstable ladder and bear their weight on the top and top step of a step ladder.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall hazards: (a) Establishment: On or about May 6, 2013, and at times prior, a worker operated Genie, Model GS 1930, serial # 33584, scissor lift without attaching the platform entry chain that forms a mid-rail prior to operating the equipment. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is an entrance to a ramp, stairway, or fixed ladder: (a) Clean Side of Production: On or about May 6, 2013, and at times prior, workers without fall protection were exposed to falling 11 feet 2 inches from the top of 1 or more dryers that were not equipped with standard railings.
