29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   The employer failed to develop and implement an adequate lockout and/or tag out program.  This was most recently evident during the inspection of the facility located at 91-188 Kalaeloa Blvd.  Kapolei, HI 96707, where the employer has employees operating and maintaining equipment with hazardous stored energy while using an inadequate written lockout and/or tag out program.  Instances include but are not limited to:   i.     The companys written program does not address employee training.   ii.    The companys program has a written general procedure but fails to address the need for written specific procedures for each individual pieces of equipment.  iii.   The companys written program does not address the requirement for annual periodic review of written specific procedures.

This was most recently evident during the inspection of the facility located at 91-188 Kalaeloa Blvd.  Kapolei, HI 96707, where the employer has employees operating the Toyota forklift to move material throughout the facility, the fork lift has an illegible data plate making it impossible for operators to verify the forklifts lifting capabilities.
