29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) Establishment: On or about 5/30/2013, the #1 Toyota powered industrial truck was being operated by employees when the driver's seat was missing large parts of the bottom of the seat, giving the driver an unstable surface to sit upon.
