Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On May 29, 2013, an employee was working from a platform with and opensided floor approximately 9 feet off the ground without the protection of a guardrail system or personal fall arrest system; thereby exposing the employee to a fall hazard.

29 CFR 1926.405(g)(2)(iii): Splices. Flexible cords shall be used only in continuous lengths without splice or tap. Hard service flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. a. On May 29, 2013, in the mechanical room a flexible cord used to energize the lighting fixtures of the temperary wiring had been spliced and repaired with electrical tape; thereby exposing employees with contact with electrical current.
