29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.     Jobsite: On or about 05/15/2013 two employees were observed working from an elevated scissor lift with the access gate secured in the open position exposing the employees to a fall of approximately 11 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     Jobsite: On or about 05/15/2013 an employee was observed performing construction activities in close proximity to the open edge of the decking and was not protected by a guardrail system, net system, or personal fall protection exposing the employees to a fall of approximately 15 feet.
