Jobsite: On or about 05/15/2013 two employees were observed working from an elevated scissor lift with the access gate secured in the open position exposing the employees to a fall of approximately 11 feet.

Jobsite: On or about 05/15/2013 an employee was observed performing construction activities in close proximity to the open edge of the decking and was not protected by a guardrail system, net system, or personal fall protection exposing the employees to a fall of approximately 15 feet.
