29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:      a) On or about May 24, 2013, the employer did not ensure that employees were using proper personal protective equipment.  Roofers were installing shingles with pneumatic nailers without using adequate eye protection.                                Pajaritos #2 Inc was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 312142508, citation number 1, item number 1 and was affirmed as a final order on 8/18/2009, with respect to a workplace located at 6338 Valdai Court, Centennial, CO      Abatement Note:   Abatement certification and documentation are required for this item. 29 CFR 1926.501 (b)(13) Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.     a) On or about May 24, 2013, employees were exposed to fall hazards in excess of 9 feet, while installing a new roof without fall protection.  The employer did not enforce the use of a fall restraint or arrest system nor did the employer implement an alternative fall protection plan to address the higher hazards at 16 feet that also existed.         Pajaritos #2 Inc. was as previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 31242508, citation number 1, item number 2 and was affirmed as a final order on 8-18-2009, with respect to a workplace located at 6338 Valdai Court, Centennial, CO.     Pajaritos #2 Inc. was as previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 312142458, citation number 1, item number 2a and was affirmed as a final order on 9-30-2009, with respect to a workplace located at 11592 Larkspur,  Parker, CO     Abatement Note:   Abatement certification and documentation are required for this item.
