29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures shall follow recognized and generally accepted good engineering practices. On or about May 22, 2013, the employer operated four Coker unit drums (D-1, D-2, D-3 and D-4) in continuous service for 55 years, and two Coker unit drums (D-10 and D-11) in continuous service for 29 years. All six drums had a well-documented history of frequent and on-going mechanical problems and repairs. The employer had not assessed the drums according to the 2007 ANSI/API/579-1/ASME "Fitness-for-Service" Standard ("Level 3"). The employer did not confirm that the drums were safe to continue in-service within the limits of critical operating parameters, with stipulated and appropriate on-going monitoring of their mechanical condition, and necessary repairs, for a determined period of time. Alternatively, the employer did not determine whether any or all of the drums should be retired or replaced. Employee(s) were thereby exposed to the hazards associated with fires and explosions.
