29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use.   On or about May 28, 2013, a Caterpillar powered industrial truck, model FL01, having serial number C674116, was used to elevate employees on a work platform not designed by the manufacturer, exposing the employees to an unprotected fall to the ground below.
